Clinical Nurse (Renal)

Who We Are:

Responsibilities:

·         Responsible for promoting and maintaining high standards of professional nursing care by providing quality services with special focus on Care of renal patients.
Qualifications:
·         Diploma in Community Health Nursing (KRCHN)
·         Higher Diploma in Renal Care from a recognized institution will be an added advantage
·         Registration Certificate issued by the Nursing Council of Kenya;
·         Valid Practicing License from Nursing Council of Kenya
·         At least two years of experience in renal care nursing
·         Certification in BLS or ACLS will be an added advantage

Pharmacy Coordinator
Who We Are:

Responsibilities:
·         Deliver drug discharges to patients in the ward, for the effective provision of pharmaceutical services.
·         Receive prescriptions from patients, verify and dispense drugs within approved standards and procedures to enable the overall accomplishment of the departmental goals.
·         Counsel patients on drug administration for optimum customer satisfaction.
·         Compound drugs when required, for effective service provision.
·         Verify expiry dates of drugs prior to dispensing to prevent anomalies in service delivery.
·         Compl with and observe the environmental health and safety measures and regulations.
·         Dispense drugs accurately and in a timely manner to uphold quality standards of the department.
·         Ensure the accuracy of the label before dispensing for the right patient, right drug and right quantity, for effective service delivery.
·         Maintain and update personal drug knowledge data base to ensure the accurate provision of pharmaceutical care of general disease states as well as specialized knowledge in the assigned area of expertise.
·         Assisting in facilitating and act as a secretary in PTC meetings
·         Conduct training to Pharmacy staff in related to CPOE or any changes or updates.
·         Ensuring stocks adjustment forms are filled, signed and adjusted by finance in a timely manner.
Qualifications:
·         Certificate in Pharmacy
·         5 years’ experience as a dispenser
·         Knowledge of the hospital management information system ( CARE 2000)
·         Computer literate, good communication skills
·         Familiarity with hospital pharmacy functioning and locally available drugs and their indications.
·         Ability to keep up with Continuous Pharmaceutical Developments and Continuous Medical Education programmes.

Clinical Pharmacist
Who We Are:

Responsibilities:
·         To offer drug information to consultants, physicians, nurses, and other users of the hospital
·         To participate in the review and update of the Aga Khan Hospital, Mombasa, Formulary to enable optimal drug usage
·         To serve as an active member and to give professional advice to the Pharmacy Department, Pharmacy and
·         Therapeutic Committee, Infection Control,
·         Antibiotic Stewardship and other Advisory Committees as appropriate
·         Attend doctors rounds in ICU and medical ward
·         Conduct face to face and other modes of medication reviews and agree medicines care plans in agreement with patients and carers
·         Organize and oversee repeat prescribing, repeat dispensing, electronic prescribing and medication review systems

·         Promote a culture of constant improvement and excellence
·         Conduct regular clinical audits and other improvement activities
·         Prevent, detect, resolve and document medication-related problems and make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team
·         Attending mandatory CMEs, conferences, and trainings
·         Participating in departmental education activities.
Qualifications:
·         Master’s Degree in Clinical Pharmacy
·         At least 2 years working experience in a similar position, preferably in a busy hospital set up
·         Registered with the Pharmacy & Poisons Board
·         Experience with an online prescription system is a plus
·         S/he must demonstrate an understanding of quality initiatives not limited to JCIA and ISO
·         S/he should have strong leadership, people management, negotiating and influencing skills with the ability to build relationships and encourage consensus, partnering and networking at all levels.
·         Demonstrated critical thinking skills in the problem-solving process.
·         Ability to organize workload and identify priorities appropriately.

Section Head – Biochemistry Services
Who We Are:
The Aga Khan Hospital, Mombasa is an institution of Aga Khan Health Services, Kenya which is an Agency of Aga Khan Development Network. The hospital is part of an integrated health system that includes Hospitals and outreach health centers across East Africa.
Responsibilities:
·         Perform tests according to standard laboratory procedure and guidelines.
·         Monitor turnaround time of results and manage referral specimens to other hospitals
·         Responsible in all sections with the assistance of assigned Laboratory Technologist
·         Conduct competency evaluation test for Laboratory Technologist
·         Ensure maximum and cost-effective utilization of reagents and appropriate stocking of reagents and consumables through CARE 2000
·         Participate in continuous medical education programmes of the department and regularly update the technologists with new and existing techniques
·         Supervise the daily performance of laboratory technologists assigned to the section.
·         Ensure compliances of all the daily quality assurance requirements in the sections
Qualifications:
·         Higher Diploma or BSCI in Medical laboratory sciences
·         Registered and licensed by KMLTTB
·         Minimum of Six (6) years working experience as a laboratory Technologist
·         Minimum of Four (4) years working experience in Biochemistry.
·         High degree of administrative capability and good leadership skills
·         Ability to write and review SOPs
·         In-depth knowledge regarding hospital-wide quality initiatives ISO 15189 and JCIA will be an added advantage.

Pharmacist
Who We Are:
The Aga Khan Hospital, Mombasa is an institution of Aga Khan Health Services, Kenya which is an Agency of Aga Khan Development Network. The hospital is part of an integrated health system that includes Hospitals and outreach health centers across East Africa.
Responsibilities:
·         Dispense prescriptions accurately.
·         Communicate with prescribers any time a prescription order is unclear or potentially harmful for a patient.
·         Ensure patients’ safety by checking for potentially dangerous drug interactions.
·         Counsel patients on proper medication use and their potential side effects
·         Giving drug information to user departments
·         Checking pharmaceutical stock to determine inventory level
·         Anticipating needed medications and supplies and placing and receiving orders from stores
·         Collecting data on Medication errors, near misses and interventions and submitting on a monthly basis.
·         Perform administrative tasks including proper documentation, adequate inventory management and generation of adequate reports
·         Setting and enforcing workplace policies
·         Keep up to date on drug approvals, product recalls and ensure compliance with regulatory authority.
·         Educate doctors, nurses and other healthcare providers about new medications and drug therapy protocols.
Qualifications:
·         Bachelor’s Degree in Pharmacy
·         2 years’ experience in a Hospital, retail or industry pharmacy
·         Registered with the Pharmacy & Poisons Board
·         S/he must demonstrate an understanding of quality initiatives not limited to JCIA and ISO
