The Home & Community Based Care Programme is a Diocesan programme covering the larger districts of Nakuru, Naivasha, Baringo and Koibatek.
It supported by Catholic Relief Services and mandated to work with People Living With HIV/AIDs through the ‘family centered approach’.
The main activities are geared towards strengthening the community in regard to agriculture, psycho-social support, economic support and health Care.
Reporting to HCB Coordinator, the person will implement the HBC/OVC programme in Nakuru Central District
Duties & Responsibilities:
• To collaboratively plan and implement the HBC/OVC programmes in Nakuru Central District.
• To supervise, monitor and evaluate work performance of ToTs and CHWs in your area.
• To promote and enforce confidentiality in all aspects of HIV/Aids programme within the Diocese
• To collaboratively plan and facilitate seminars, workshops and training sessions with the DHBC
• To demonstrate accountability for work time by accurate and up to date documentation of daily activities
• To network and liaise with MOH and other stakeholders
• To ensure end month reports are completed and submitted to Data Clerk
• To monitor and evaluate income generating activities of the support groups
• To maintain up to date inventory of supplies and equipments being used in your area
Qualifications & Experience:
• Must have atleast Diploma in Community Development, Social Work or any other related course
• Must have experience of atleast 2 years in HIV/ Aids & OVC programmes
• Must have knowledge and experience in community participatory approaches
• Must have a valid driving license of classes BCEFG and especially competent in driving a motorbike
• Must be computer literate
