Position Summary:

This position contributes to SEE goal “Transformed individuals, families and communities through spiritual, physical, economic and social empowerment who embrace progressive values, and engage in productive, gainful and sustainable activities’’.

Key Responsibilities:


Case manager will implement OVC case management based on PEPFAR OVC case management standardized approaches and tools for identification, assessment, case plan development, service provision or referral, monitoring, graduation and case closure

Case manager will support provision of OVC interventions within the four OVC domains of Healthy, Safe, Schooled and Stable to ensure; timely service provision, graduation and transition to resilience for OVC and their families.

Case manager will build capacity of community workforce (case workers, LDP and EBI facilitators) and report on all project activities related to case management and service delivery as per the aspirations of the USAID Tumikia Mtoto project, whose aim is to ensure improved access to health and social services for OVC and their families in collaboration with public and private service providers.

Case Manager will monitor and report on activities in the strategic workplan related to case management and service delivery with respect to OVC and their families.

The department OVC service-oriented approach will ensure that the project consistently applies Case Management approach to the delivery of quality and time bound services to the OVC and their families as well as enhance their participation in their best interest to constantly work towards improving the impact of the benefits to the beneficiaries.

Case Manager will be reporting to the team leader, work and coordinate closely with key staff from the relevant ministries, Community Resource Persons; (Community Mother Mentors, Community Health Volunteers and Link Desk Persons) to contribute to a holistic, integrated, and comprehensive project implementation.

Case manager will conduct joint support supervision with relevant GOK representatives to forge better working relationships and leverage.

Case manager will conduct regular home visits jointly with the community workforce to monitor progress, ascertain service provision to the project beneficiaries and provide support to the volunteers.

Case manager will lead onsite monthly review meetings with project volunteers to ensure they also reflect on project experiences for purposes of improving service delivery.

Knowledge

Demonstrate Knowledge and experience working with vulnerable individuals, families and communities.

Relevant experience in Community development project monitoring, evaluation, accountability and learning.

Excellent computer skills; skilled in MS Office Suite including Word, Excel, PowerPoint and Access.

Excellent report writing skills.

Functional/work-based skills

Ability to cultivate constructive relationships.

Strong interpersonal and public relation skills and able to work with people from diverse back grounds.

Able to work independently and effectively under pressure.

Excellent verbal and written communication skills

Familiarity with rural/urban working environment

Knowledge of OVC case management methodology

Strong coordination skills and strong team orientation.

Strong community mobilization skills

Strong networking skills.

Ability to use computers.

Qualifications

Bachelor’s degree in Social Sciences and other related disciplines, at least three (3) years’ experience in a similar environment

A minimum of 3 years demonstrated experience in implementation of OVC case management

Ability to ride motorcycle with valid riding license will be an added advantage
